Policies

  • Upon entering a trainer/client agreement, client’s will be expected to complete and sign the following forms:

    • Personal Training Contract/Agreement

    • Waiver and Release of Liability

    • 2020 Par-Q+ (The Physical Activity Readiness Questionnaire for Everyone)

  • The client must disclose any physical conditions, impairments, diseases, or illnesses that might affect or prevent the client’s participation. The client will notify the trainer of any changes to their health or medical condition and will only participate in training sessions with the consent of their physician.

  • If the client purchases a package of training sessions, payment for the package will be due in full prior to the commencement of the first training session. If the client purchases one training session at a time, payment per training session is due in full prior to the commencement of each training session. Payment for Bootcamps is taken when booking in order to reserve your spot in class.

    The following payment methods are accepted: cash, check, credit/debit, and Venmo (@FITNYS).

  • Personal training sessions must be cancelled at least 24 hours in advance. Sessions canceled less than 24 hours in advance will be charged in full to the client.

  • Clients arriving late will receive the remaining scheduled session time, unless other arrangements have been previously made with the trainer.

  • Personal training sessions are NONREFUNDABLE. Client may have up to 1 year to complete purchased sessions.
